Scheduling an appointment

You can make an appointment by calling your clinic or coming in-person during regular business hours. Please have the following information ready when you make your appointment:

  • Name and birth date
  • Whether you are a new or returning patient
  • Daytime phone number
  • Reason for appointment
  • Preferred date for appointment
  • Language (if you need an interpreter during your appointment)

Canceling an appointment

It is important that you notify us when you are unable to keep your appointment. If you need to cancel or reschedule an appointment, please call us at least 24 hours before your scheduled appointment time. This will allow us to offer your appointment slot to another patient. It will also give us enough time to cancel interpreter services we may have arranged for you.

What to bring to an appointment

When you come to your appointment, it is important to always bring the following:

  • Appointment card and your insurance or ProviderOne card
  • Co-payment
  • Proof of income (if you wish to apply for the sliding-scale fees)
  • Medication bottles or a list of your medications and their dosages
  • List of supplements or herbs you are taking and their dosages
  • Immunization records
  • Signed permission slip from a parent or guardian if a child is accompanied by someone other than their parent or legal guardian
  • Current medical records, if you are a new patient or have not received care from ICHS in past three years
  • Contact information for previous health care provider, if you do not have current medical records.

After your appointment, please return to the reception area to check out. A staff member can help schedule a future appointment or refer you to other services, such as a lab test, behavioral health, nutrition information or health education. At a dental appointment checkout, a staff member can collect any payment or give an estimate of the next visit’s cost.

Information for patients under 18

If you are under the age of 18 and need services such as family planning or sexually transmitted disease (STD) diagnosis and treatment, you are not required to obtain parental permission to receive care.

For all other services, patients under the age of 18 must have permission for their parent or legal guardian to receive treatment.
