    Job Description
    The Registered Nurse plans, organizes, and implements nursing services to patients in conjunction with providers.

    Position Requirements

    • Education – Completion of a BSN, or an AND, from an accredited institution.
    • Experience – One year ambulatory nursing experience preferred, in areas such as public health, community health, or equivalent and experience in multicultural nursing or other social service area and commitment to community health care preferred.
    • Other Requirement(s) – Current WA State Registered Nurse (RN) license. Basic Life Support (BLS) for Healthcare Providers.

    Licensed Practical Nurse

    Job Description

    The Licensed Practical Nurse will provide skilled nursing services in the PACE clinic and support the transition of care for the PACE participants. This will include following up on patients post hospital discharge, post ER visits, post procedures, post-surgery and follow up on any acute changes or symptoms which may arise via patient phone calls and support the program goals to avoid readmission to hospitals and skilled nursing facilities. 

    This position will help to manage all aspects of the care from inpatient stay to home. This role will: identify high-risk, complex patients and communicate with all entities involved in the care of the patient to promote and maximize care coordination; uses all components of the transitional care model and nursing process including assessment, triage, planning, implementing and evaluating care to meet the patients’ needs; and plans, organizes, and provides nursing services to clinic patients in conjunction with clinic Providers and Registered Nurses.

    Position Requirements

    • Education - Completion of a Licensed Practical Nurse program.
    • Experience - A minimum of one year of LPN experience is required, along with proficiency in utilizing electronic health records. Experience with the geriatric population is preferred.
    • Other Requirement(s) - Active Washington State LPN license. Fluency in English and any Asian Pacific Islander languages preferred.

    Registered Nurse - Care Management

    Job Description

    The Registered Nurse - Care Management plans, organizes, and provides nursing services to clinic patients in conjunction with clinic providers.

    This position works collaboratively with providers and other members of the health care team to improve the health of patients with chronic disease conditions and/or complex needs; educates patients and families to help them manage their health care needs; facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management.

    Position Requirements

    • Education - Completion of a BSN, or an ADN from an accredited institution.
    • Experience - Two years ambulatory nursing experience with BSN or three years with an ADN preferred, in areas such as public health, community health, or equivalent and experience in multicultural nursing or other social service area and commitment to community health care preferred. Care management or chronic disease management experience preferred.
    • Other Requirement(s) - Current WA State Registered Nurse (RN) license. Basic Life Support (BLS) for Healthcare Providers. Certified Diabetes Educator (CDE) preferred. Obtaining specialty certification in care management within 12 months of hire required.

    Wellness Nurse Manager

    Full-Time • $99,840.00 To $146,175.00 annually

    Job Description

    The Wellness Nurse Manager is responsible for the supervision and coordination of activities related to nursing services within an assigned living facility. This position oversees and ensures in meeting patient care requirements in accordance with physician instructions, and in compliance with federal, state, and local laws and agency policies and regulations.

    Position Requirements

    • Education - Completion of a BSN, or an ADN from an accredited institution.
    • Experience - Five (5) years nursing experience with BSN or/and an ADN with three (3) years of those years in a leadership role in Assisted Living or nursing home setting required. Experience in multicultural nursing or other social service area and commitment to community health care preferred.
    • Other Requirement(s) - Active WA State Registered Nurse (RN) license. Basic Life Support (BLS) for Healthcare Providers required. Specialty Training (Dementia and Mental Health) and DD training upon hiring. Ambulatory certification within 12 months of hire required.

    Nina’s Story

    Nina Zhang has been assistant nurse manager with International Community Health Services (ICHS) for a little over a year now.

    Before joining ICHS, she was an inpatient nurse, working in a large hospital, caring for patients in serious condition. For Nina, she moved to primary care to find a healthier work life balance, but also to learn and grow.

    At ICHS, Nina has thrived.

    Today, you can find her at ICHS’ International District Medical & Dental clinic.

    She is on a team of highly qualified primary care nurses serving patients of all ages and backgrounds. Daily, she coordinates nursing services with behavioral health specialists, nutritionists, physicians, and others.

    It’s the team environment and supportive supervisors that have inspired her to grow and thrive.

    “My manager is such a supportive person to work with, she’s always there to listen. She offers really good advice. And in her role as care manager she has so much knowledge, and is always willing to teach others, offer advice, and constructive criticism. That’s great because all of us on our team want to grow.”

    At the end of the day, she reflects upon the patients and the way she's able to build relationships with patients that feels most meaningful.

    “We have the ability to form longer and more meaningful relationships in primary care, we see our patients throughout the year, all throughout the years we see patients who end up having their kids here, and later on we see their kids. We see generations go through.”

    Emily Brothers launched her nursing career at ICHS in 2018 as an OB nurse, “straight out of nursing school.”

    Passionate about health access for all, she joined ICHS as a primary care nurse to empower, not only treat, her patients. “[My goal was to] find solutions to their health conditions throughout the social determinants of health.”

    At ICHS, Emily has thrived.

    Today, she is the Assistant Nurse Manager at ICHS’ Shoreline Medical & Dental Clinic, ICHS’ newest clinic location.

    In the integrated care model at ICHS, Emily has worked in small close teams with her colleagues across departments, and learned a great deal from mentors and teammates.

    “I see so many different patient cases in so many different areas of medicine every day. I'm constantly, constantly learning, and I really love that. It keeps you humble, but it's really energizing too.”

    At the end of the day, she reflects upon the team and mentors at ICHS that keep her going and inspire her. 

    “There's burnout in a lot of healthcare settings. I really appreciate that I’m working with people who continue to care and to give it their all.”
